Getting Started with Mobility Equipment in Charlotte

In order to provide you with a seamless path to freedom and independence, we have a simple, five-step mobility process. This allows us to provide you with more personalized solutions so you have the exact tools to suit your unique needs!

Step One: Mobility Exam

Your physician or therapist will need to perform a thorough examination to quantify your mobility limitations and provide the necessary documentation to secure your mobility device. We will work with you and your physician regarding any insurance requirements to minimize your visits and complete the process quickly.

If you are currently working with a home health Physical or Occupational Therapist, have them send us your insurance and demographics information.  We will verify that we are in-network with your insurance and schedule the mobility evaluation with your therapist.

If you are not currently working with home health, you will need to have your physician send us an order for “Physical or Occupational Therapy to evaluate for mobility needs by home health or in a wheelchair seating clinic”.  Once we receive the order and verify your insurance, we will coordinate and schedule your evaluation.  Click Here to access our order form.

 

Step Two: Complete the Paperwork

Upon completion of the exam, we will send your physician the remaining paperwork to sign.  Your physician may request that you come back in for a face-to-face visit for a mobility evaluation, if you have not previously discussed your mobility needs with your physician within the last 6 months.

Step Three: Home Evaluation

We will visit your home to assess your doorways and thresholds to ensure that the power wheelchair or another device can be adequately maneuvered through your living environment.

Step Four: Pre-Authorization (Custom Manual and Power Mobility Only)

After your evaluation, we’ll go over the information and prepare and submit documentation to your insurance or to Medicare for authorization. Medicare’s clinical review team has up to 10 business days to process the request; once completed, a decision letter will be mailed to all parties involved.

Step Five: Delivery

Our Patient Service Technicians will deliver your wheelchair and customize your equipment to your needs. We want you to experience comfort and convenience, and we understand that it may take some time to get acclimated to your new chair. We are here to help!
